New Patient Registration
The practice only accepts applications from patients who are living within the designated practice boundary
Each new patient is given the opportunity to undergo a medical check-up. This initial check-up is carried out by the nurse and helps her to assess your general health and to familiarise them with your medical history (please bring a urine sample with you). Children under five do not need to attend unless you so wish.
Change of Address
Please let us know as soon as possible any change of name or address. Unless you do this, the Family Health Service Authority may remove your name from you list of patients without letting you know. Please note the catchment area for the surgery covers Allington postcodes only.
Facilities
Our surgery premises have been designed to allow disabled access. All our services and facilities are available on the ground floor. We provide a children’s area and ask parents to consider other patients by ensuring their children have put away the toys when they have finished playing so the accidents can be avoided.
